PVR350 + SB 24bit Live: Banging my head against a wall

Post subject:  PVR350 + SB 24bit Live: Banging my head against a wall
Hardware:
AMD K2 w/ 192M
PVR 350 used as display. MPEG2 decoding turned on.
SB Live! 24 Bit used with powered analog speakers. This card was installed after Knoppmyth and alsaconfig was used to set it up.

What is working:
TV Out
Playing CDs
TV Display
Zap2It etc

So I'm close

What I'm trying to fix:
1) I cannot adjust volume when playing CDs. The volume setting goes up and down but no change to volume. Settings:
Use internal volume control is checked
Mixer device is /dev/mixer, mixer Controls PCM (using Mixer made no difference)

2) Don't get TV sound when PVR350 is routed to the sound card. I do get sound out of the 350 when I plug its sound into the TV.
I have have the 350 sound going into the line in connector on the SB.
I have tried setting up a .asoundrc in Myth $HOME based on alsa docs for the card. Not sure if this is a piece of the puzzle or not.
