Sound has been working perfectly on R5d1 for months. I think this may have happened on a reboot via commandline (not just powered down), however I'm not sure. I have tried unmuting everything via alsamixer and the mute on the remote doesn't control this. I'm using a pvr350. The following shows up in the top right side of the screen when watching TV:

Anyone have any idea how to get rid of this? On the left side is a speaker with an x through it. Not sure if this means it's muted somewhere or if there is a hardware problem. Hopefully it's just a setting that I'm overlooking. Any help is greatly appreciated.

Have you tried the mute button on your remote or the F9 key on your keyboard?

Yes, I tried both of those and they don't seem to control this box on my screen. When they are used there is text that pops up on the top left side of the screen showing muted or un-muted.

I don't recognize that control from any of the OSD themes I've tried. Are you sure it's not from the TV itself?

I'm sure it's not the television controls. If I watch a recorded show the sound works fine (prior to this problem staring). Since this problem started happening any recordings have this osd and no sound.

As you are using a PVR350, am I correct that you are using a set top box as your video source? If you are, it could be that your STB has been muted accidentally, if you unmute your STB, you should be able to to get sound on new recordings. Of course, previous recordings will still be muted. :)

Bingo, it was my stb muted. I didn't even think about the stb being muted. Thank you guys for your assistance!
