I am using a PVR-500 and have had previous problems on the install:
So far this is what I have done to install the PVR-500 and ivtv:

untar kernel
tar jxvf /usr/src/KERNEL-VER

Note: the tuner.ko and tveeprom.ko that reside in the
/lib/modules/KERNEL-VER/v4l2/
and
/lib/modules/KERNEL-VER/ivtv
RENAME all the tuner.ko and tveeprom.ko and msp3400.ko to .DELETE

wget new driver:

tar -xvzf ivtv-0.4.0.tar.gz
cd ivtv-0.3.5f/driver
make
make install
cd ../utils
make
make install
reboot

I can hear sound find when I play a cd/mp3.
I already checked alsamixer and dont think that it has anything to do with it, because I can hear sound from mythtv being played back fine.
I also made sure the volume was up on LiveTV and not mutted.
I assume that it is not decoding it from the coax line correctly.
What problems might be occuring? What logs should I look at to check?

This is a known bug with the IVTV driver. I use version 3.8 with no problems. I've tried 4.0 stable but it gave me the same no-sound problems.

how can they call .4.0 stable if it does that then!? -- HOW rediculous!

I used 3.9 with no issues, before 4.0 came out. Did you make sure to power down your machine, or did you just reboot? Rumors are it makes a difference. I didn't take the chance, and just powered down.
