I have plan to switch my r5d1 from EPIA10K (cle266 + unichrome) to p5p800 + GF5200.
People, according to Your experience - will following procedure be enough:
1.change xfree to VESA (driver statement from "via" to "vesa")
2.move hdd to new hardware
3.boot and launch r5d1 nvida install script

Is this all required, or I have do something additional for xvmc, etc ?

There should be no reason to run the nvidia script as the system should detect the card and install the driver.
