When I watch live TV, I hear everything perfectly but get no video. The weird thing is, when I go to Watch Recordings, the video shows in the little preview box.

Any idea why I would get the video in the small box but not fullscreen on my TV?

You haven't given much info here, but...

I'd try disabling XvMC and/or opengl if they're enabled.

Tell us the result.

I was able to get the TV function to work by changing the device from /dev/video16 to /dev/fb0. TV works now, but I can not watch my movies from media library anymore. Is there any way to make mplayer use /dev/fb0 instead of /dev/video16?

I have the same issue where there is audio but no video. I can see videos in preview panes also. I tried moving to fb0 and that was able to bring up video, but then no audio.
After moving back to /dev/video16 and messing around with the gui offsets I saw that when I did watch TV the background which is not in the GUI, the KnoppMyth Desktop area, shows the video, but the GUI area only shows black.
I tried disabling OpenGL verical synv and XV picture controls but it comes up the same way.

I am using a PVR-350 and using the video and audio out from that card.

5.4) The new drivers also mean changes to the PVR-350 output. If you have problems you may need to add the following line to your /etc/mythtv/modules/ivtv file as per the IVTV Troubleshooting page

I had this same problem and needed the same fix. I guess it's common with the PVR-350? For me the OSD overlay worked, but regular TV watching only showed flickerings around the borders (where I had the window offsets configured), and a huge black square in the middle.
